It rule adds chance to unintentionally kills enemy while trying to take him alive.
Makes sense. It stresses how potentially dangerous a knockout blow is, how hard it is to actually beat someone into unconsciousness, and how much you are at risk of actually killing your opponent rather than subdue him/her.

I have given much thought to the subject, and although I have not managed to come up with a perfect solution rules-wise, I now dictate that the only way to actually put foes out of action is to 1) Remove all of their Health points, which, no matter the method used, will potentially kill them (see the "Agony" rules on p.239) 2) Restrain them, whether by seizing them or by using rope.

In the end, I don't think there could exist a perfect rule for "knockout blows", nor that it would be very realistic, since when you knock someone out, they actually suffer brain damage of a potentially very severe nature, which may lead to death or medical issues.

That's generally what I've been doing, too. How do you adjudicate attempts to restrain someone? I have just been using opposed Feats rolls for any sort of action like that: Restraining someone, tackling them to the ground, shoving past someone blocking your path, etc.

Same for me: Feats or Close Combat vs Feats or Close Combat (both parties are free to use the Domain most advantageous to them). Simple and straightforward!
