There are some basic rules for it in book 2 travels. It functions similar to miracles

Yeah but for some, getting maimed is worse than char death. A warrior that loses use of a hand for example. Or a varigal that loses his legs.

The premade characters in travels i believe are primarily NPCs with a long history. If i remember correctly they are much stronger than anything right out of character creation.
